There's like 450 questions for FM and like 1000+ for "Medicine". I felt like it had a good spread of systems like you mentioned but it tended to be more outpatient management oriented. FM comat is easier. Lots of guideline based questions like what age to do colonoscopy and stuff. FM was a bit easier and it was more outpatient based.

I did the same thing for every single COMAT. I did 100 new cheesy dorian a day for the topic (ex IM or FM or OB plus whatever previously done cards). And then in the last three weeks I took the comquest questions for the comat and divided it by 18 days and just that many questions a dayThey are pretty close. I was always within 1-2 pts from the prediction either way. For IM I scored higher than my prediction. I will suggest that you just switch to UWorld though. It is a better qbank and I did much better on the COMATs that I used UWorld on. Averaged 100-105ish using comquest. Averaged 110+ using UWorld. Good luck on your COMAT!
